Use a tipped tube
Use an extra-Long rat tooth forceps to grab tip of jejunal tube in the stomach
Advance tube into the duodenum and past the ligament of Treitz by gently pushing both scope and forceps (with tube).
Bring back scope to stomach, leaving forceps with tube in deep duodenum.
Open forceps and bring it back to stomach, while tube remains in duodenum.
When removing scope and forceps out of the stomach, through the esophagus, you may use some insufflation to increase the esophageal lumen and thus, prevent pulling the jejunal tube out.
Alternatively, you may use the rat tooth forceps to grab the tube located in the stomach and pull the scope out, so that the scope friction does not pull out the jejunal tube.
Once the tip of the scope is at the upper esophagus or hypopharynx, you can open the fat tooth forceps and remove it.
